Going Home

Early in your hospital stay, our discharge planner will be in touch to ensure a smooth transition when you go home. Our discharge planner will help to plan for services you may need when you leave the hospital.

The hospital discharge time is 10 am. Please arrange for a family member or friend to take you home after your hospital stay.

When you leave, please make sure that you have all your belongings, as well as any instructions to follow when you get home and any prescriptions.

It is very important that you understand the following before you leave the hospital:

  • Your medications and what to eat and drink

  • When and how to return to normal activities

  • The date and time of any return for follow-up appointments

Services from the Champlain Community Care Access Centre (CCAC) may have been arranged for you once you are home. If you have any questions regarding these services, please contact CCAC directly at:

  • 310-2222 (no area code required)

  • Toll Free: 1 800 538-0520

  • Outside Canada: 613 745-5525

If you are unsure about anything, please ask your healthcare provider before you leave. We wish you the best of health.
